Notification of Infringement – Claim

- Contact information of the complaining party including name, address, email, phone number, and fax number;
- A statement that the use of the material is unauthorized by the copyright owner;
- A declaration that the information provided is accurate, and that the complaining party is authorized to act on behalf of the copyright owner.
Section 17 USC §512(f) outlines penalties for misrepresentation in infringement claims under 17 USC §512(c)(3), including legal costs and attorney fees.

Counter Notification – Restoration of Material
If you receive a takedown notice due to a copyright claim, you may send a counter notification to have the material restored on the website. This notification must include:

- A description of the material taken down and its original location;
- A statement under penalty of perjury that the material was removed due to mistake or misidentification;
- Your name, address, telephone number, and consent to jurisdiction of the court.

Policy on Repeat Infringers
